Notice Regarding U.S. Courts Forms

The Federal Judiciary maintains a current listing of all forms used in the Federal Courts. Attorneys and litigants are advised to first use locally developed forms, when available. In the absence of a local form, attorneys and litigants are to use the most current version of the federal form.

Note on Disbursements

When the proposed Order to Disburse requires payment of interest to a payee or payees, the filer must e-mail to  an AO 213P form completed and signed by each payee receiving funds from interest earned.

Deposits of Security/Non-interest Bearing

Use this template for deposit of security pursuant to Rule 65, Admiralty Rule E or F, or for Rule 67 deposits waiving deposit into an interest-bearing account.
