NOTE: PDFs must be text-searchable and PDF fillable forms must be flattened or otherwise unmodifiable prior to attachment.

To open a civil case, click on the Civil menu, and then select Open A Civil Case under the Open a Civil Case – Attorney heading.

Requirements for Filing Patent, Trademark, or Copyright Cases

Pursuant to LR 10-4(a)(2), at the time a pleading initiating an action that includes a patent, trademark, or copyright claim is filed, a completed AO 120 (patent and trademark) or AO 121 (copyright) form(s) and must also be filed as a separate docket entry. The AO 120 or 121 form is to be filed using the Notice of Filing – Copyright, Trademark, or Patent Information. The event will inquire whether the form filed is the AO 120 or AO 121 during docketing and will require the filer to relate the filing to the pleading initiating the action. The Notice of Filing – Copyright, Trademark, or Patent Information is also to be used to file the AO 120 or AO 121 form for copyrights, patents, and trademarks subsequently added as well as to file the AO 120 or AO 121 form within 10 days of the final decision or judgment on the copyright, trademark, or patent.
